SJU Health Survey Introduction

2018 SJU GENERAL HEALTH SURVEY

Thank you very much for participating in this survey. Filling out the general health survey is completely voluntary. It should take about 15 minutes to complete. There may be some questions that may be considered sensitive. Be assured that your responses are anonymous. Your name or e-mail address will never be associated with your responses. You may omit answers to any questions you so choose and you have the right to not participate at all. Your honesty is appreciated and is valuable in obtaining accurate results.

In appreciation of the time you spend filling out the survey, you will have an opportunity to win one of many Target gift cards, worth between $50 and $200. After you submit your completed survey, you may fill in a 2nd short form, which will make you eligible for the drawing. This second form goes to a completely different data base than the survey information.
Please note that due to tax laws, if you are a student employee and win a gift certificate, you will be taxed on the prize.

This survey has been reviewed and approved by the Institutional Review Board (IRB) of the College of St. Benedict/St. John's University.
