Please complete this form as completely and as detailed as possible. It usually takes 2-3 business days for submissions to be completed. If you need to request the video recording of an upcoming event, please submit our A/V Support Request Form instead to request media support.
There are legal restrictions relating to copyrighted materials and specific procedures must be followed to comply with the law. Submission of this form does not automatically guarantee service. It is the responsibility of the requestor (you) to ensure the proper permission requirements have been met for each video included. We reserve the right to deny your request if we determine that additional permissions need to be acquired or if the request cannot be completed (in whole or in part) due to copyright restrictions. Digital Commons is a public repository service of the College of Saint Benedict and Saint John’s University Libraries for content produced by the students, faculty, and staff from departments and centers at our schools. Before a video can be placed on Digital Commons, a release form (provided in the form upon selection) must be filled out and signed by each presenter. A signed release form is not required for current CSB and SJU students, staff, faculty and monastic members. Instructional Technology will be unable to upload the completed video to Digital Commons until we receive all signed forms in digital or printed format. There is an option later in this form to submit digital release forms with this request.
