In the event, that a student does not meet the 30-day deadline for appealing an admissions decision, he/she will need to reapply to the department. All department admissions requirements must be met.
Additionally, the student will need to address the following items for the admissions committee and submit this form to the admissions committee, due by the deadline for admissions.
