Incoming First Year students must submit both this form and the Housing Documentation form by 07/01/2025 in order to be considered for priority placement due to disability. Materials received after the July 1 deadline will be evaluated on a rolling basis.
Introduction:
The Housing Accommodation Request Form describes the process by which CSB+SJU students with disabilities can request housing accommodations. Please review this form carefully, as it includes important information about our process. This form is intended for current CSB+SJU students who have not previously been approved for housing accommodations.

Students previously approved who wish to add or modify requests must complete this form and provide updated documentation.
The College of St. Benedict and St John’s University are dedicated to providing equal access in residential living spaces for those individuals with documented disabilities. We recognize the importance of providing appropriate and reasonable accommodations when barriers exist within the residential environment. CSB/SJU students with documented disabilities must request special housing accommodations within the context of the policies stated below.
Eligibility and Documentation of Needs:
Federal disability legislation requires CSB+SJU to provide reasonable housing accommodations for individuals with documented disabilities. The Americans with Disabilities Act defines an individual with a disability as “a person who has a physical or mental impairment that substantially limits one or more major life activities.”
When the need of an accommodation is not readily apparent or known, federal housing legislation allows housing providers to request additional supporting medical documentation of a resident, to help evaluate the appropriateness of a request. Factors involved in this decision will include:
- The nature and severity of the disability
- The duration or expected duration of the disability
- Whether the disability is characterized as permanent or long-term
Supporting Documentation: Housing Accommodation Documentation Form
To properly determine how CSB/SJU can best meet the student's need for reasonable accommodations, we require the submission of our Housing Accommodation Documentation Form (link here) by a licensed clinical professional or healthcare provider that is familiar with the history and functional limitations of the student's physical or mental health condition(s). Any requests that do not include submission of this supporting documentation are incomplete, and will not be reviewed.
Submission of the form by a provider does not automatically guarantee that the recommended accommodation can or will be granted. Provider's suggestions are not prescriptions. The information shared by providers will be reviewed by professional staff in SAS. Decisions will be based on our process (outlined in this form), in consideration of state and federal housing and disability legislation and knowledge of the housing options and resources available within campus.
Deadlines
Current Students: Requests for pre-selection offers will only be considered when received on or before the Marth 20, 2025 11:59 a.m. CST deadline.
Incoming First Year Students: Please submit both this form and the Housing Documentation form no later than 7/01/2025.
Accommodation requests received from current students, for the 2025-2026 academic year, after the March 6th deadline will not be reviewed until after completion of the housing selection process. Due to busy administrative timelines at the end of the academic year, late requests received for the following academic year will not be reviewed until June. When CSB+SJU receives late requests, it cannot guarantee that it will be able to meet an individual’s accommodation needs during the academic year in question.
