Request for Optional Practical Training (OPT) Form
OPT or optional practical training, allows students to get real-world work experience related to their field of study. Students may apply for a work permit with USCIS 90 days before their Program End Date. Processing time for the EAD takes between 90 to 120 days. If the OPT is approved, USCIS will issue an Employment Authorization Document (EAD card).
Students MAY NOT work until they receive their EAD card. It is highly recommended to remain in the USA and not TRAVEL outside the country before receiving the EAD card. Students are eligible to work for 12 months and must work full-time, 20 or more hours a week.
The purpose of this form is to select an OPT Start Date, within 60 days after your Program End date. This date is not guaranteed. USCIS may select a date within or just after the 60 day period. The goal is to apply early!
OPT Application Process & Reminders:
Step 1: Students submit OPT Request form to DSO.
Step 2: DSO enters request into SEVIS and prints a new Form I-20.
Step 3: DSO and student sign the new Form I-20.
- Do not file the Form I-765 until you receive an updated Form I-20 from your DSO.
- You must file the Form I-765 within 30 days of receiving the updated Form I-20.
Step 4: Apply for an Employment Authorization Document (EAD card) electronically by creating a USCIS online account at https://myaccount.uscis.gov/. Scan all of the supporting evidence below so it can be uploaded to the online application.
- File the Application for Employment Authorization (I-765).
- Review the USCIS I-765 Instructions (uscis.gov/i-765).
- Select the basis of eligibility: (C)(3)(B) for post-completion OPT.
- One 2x2 passport photo - to be uploaded; if possible, request a digital copy of the passport photograph (purchase photos at the post office in Saint Joseph or a nearby Walgreens).
- Most recent I-94-this can be accessed at http://www.cbp.gov/i94
- Copy of Passport and visa to be uploaded (save image to your laptop)
- OPT Recommendation I-20 document/s
- Application fee - $470 can be paid by bank account (ACH), debit, or credit card
- After online submission, you will receive a receipt, Form I-797, in your USCIS online account.
Step 5: Your OPT start date begins 60 days of your Program End date and finishes 12 months after this date (one year or 365 days). The dates will be on your EAD card.
